Salary: $82,000 – $94,000 + 12% super + salary packaging

About the Company
Our client is a leading Aboriginal Community Controlled Health organisation delivering culturally safe, holistic healthcare services to Aboriginal and Torres Strait Islander communities. With a strong focus on Closing the Gap, they provide integrated primary health care, mental health, social and wellbeing services across multiple community locations.
They are seeking an Aboriginal or Torres Strait Islander-identified Health Worker to support community members with complex health and social needs through culturally appropriate, wrap-around care.

The Role
This is an exciting opportunity for an experienced Aboriginal and Torres Strait Islander Health Worker to provide outcome-focused health, social and practical support to clients with moderate to severe mental health and wellbeing needs.
Working within a multidisciplinary team, you will support clients who may be at risk of acute care admission or transitioning from inpatient services back into the community. Through intensive case management and care coordination, you will empower clients to build strength, resilience and long-term wellbeing.
Key responsibilities include:
  • Developing and maintaining individualised, culturally safe support and wellbeing plans
  • Working with clients using trauma-informed, recovery-oriented and strengths-based approaches
  • Coordinating services across mental health, alcohol and other drugs, chronic disease and social health supports
  • Conducting home, community and outreach visits as required
  • Building strong relationships with external services, agencies and community networks
  • Supporting community engagement and participation in health and wellbeing programs

What We’re Looking For
To succeed in this role, you will ideally have:
  • To perform this role, it is essential that the incumbent is an Aboriginal or Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 (essential requirement)
  • Experience working with people with complex mental health, AOD or social wellbeing needs
  • Certificate IV or higher in Aboriginal and/or Torres Strait Islander Primary Health Care, Mental Health, Peer Work, or willingness to obtain
  • Strong case management, communication and relationship-building skills
  • A trauma-informed and culturally responsive practice approach
  • Current QLD C Class Driver Licence
  • Current Blue Card, Criminal History Check, and CPR/First Aid (or willingness to obtain)
