The Role

You will provide high-quality, evidence-based social work services to patients, families and carers within a multidisciplinary environment, contributing to assessment, intervention, discharge planning and care coordination.

Key responsibilities include:
* Deliver Social Work services across acute, subacute and community settings
* Conduct comprehensive psychosocial assessments and care planning
* Provide grief and bereavement support to patients and families
* Collaborate with multidisciplinary teams and external community services
* Participate in clinical supervision, service development and quality initiatives
* Advocate for patients and support informed decision-making

You’ll work closely with a multidisciplinary healthcare team and be part of a broader network of experienced Social Workers.

What We’re Looking For

To succeed in this role, you will ideally have:
* Qualification in Social Work (Bachelor or Master) and eligibility for AASW membership
* Demonstrated experience in psychosocial assessment and care planning
* Strong communication and interpersonal skills
* Ability to work both independently and within a multidisciplinary team
* Understanding of hospital systems and community support services
