  • Up to $106k + Super + $16k Salary Packaging + Parking
  • Permanent Part Time or Full Time focusing on Client/Care Coordination

If you’re an experienced Care Coordinator / Client Manager you already understand that great care doesn’t just happen, it’s carefully planned, guided, and continually adapted.
We’re looking for someone who can combine empathy with strong coordination skills to support older Australians to remain safe, independent, and confident in their own homes. Our client is a well-established, community-focused aged care provider delivering services across home care, independent living, and residential care. As the sector continues to evolve, they are focused on creating flexible, responsive services that genuinely reflect how people want to live at home.

About the opportunity

You will manage a portfolio of clients, overseeing their care journey from planning through to delivery. You’ll be the key point of contact for clients and families, while working closely with internal teams to ensure services are effective, compliant, and meaningful. In this role, you will:

  • Build strong, trusting relationships with clients and act as their primary contact
  • Develop practical, tailored care plans aligned with current legislation and quality standards
  • Coordinate services in partnership with clinical and allied health professionals
  • Monitor and manage individual budgets to ensure effective use of funding
  • Support clients to make informed choices about their care and lifestyle
  • Respond to sector changes and adjust care approaches as needed
  • Identify opportunities to enhance service delivery and client outcomes
  • Manage budgets and support clients to navigate the ongoing SAH changes

What will set you apart

This role suits someone who is confident navigating and making balanced decisions that support both quality care and compliance.

  • Proven experience coordinating care within aged care (home or community-based strongly preferred)
  • Registered Nurse, Enrolled Nurse qualification (desirable) or Certificate IV in Aged Care or Individual Support
  • The ability to work confidently through change and evolving sector requirements
  • Relevant qualifications in aged care, community services, or a related field
  • A strong understanding of funding models particularly Home Care Packages and Support at Home program
  • Excellent communication and relationship-building skills
  • Sound judgement, initiative, and the ability to manage competing priorities
  • A genuine commitment to improving the lives of older Australians
  • Current National Police Clearance (within the last 6 months)
  • Covid and Flu vaccination
