Clinical Care Coordinator – Aged Care

- Permanent full time Monday to Friday with flexible day hours
- Residential Aged Care
- Perth Metro area – onsite parking
- Competitive Hourly Rates from $60 – $64ph + Super
 
The Role:
- Coordinate and oversee the delivery of high-quality clinical care in line with aged care standards and organisational policies
- Conduct comprehensive clinical assessments and develop, implement, and regularly review individualised care plans
- Monitor residents’/clients’ health status, ensuring timely intervention and escalation of clinical concerns
- Lead and support nursing and care staff to ensure safe, evidence-based practice and continuity of care
- Manage medication administration systems, audits, and compliance with legislative requirements
- Collaborate with GPs, allied health professionals, families, and external providers to optimise care outcomes
- Ensure compliance with Aged Care Quality Standards, accreditation requirements, and clinical governance frameworks
- Oversee and participate in clinical audits, risk management, and quality improvement initiatives
- Provide education, mentoring, and clinical guidance to staff to build capability and maintain best practice
- Maintain accurate and up-to-date clinical documentation and reporting, including incident management and care evaluations. 
  
The successful candidate will have:
- Experience with Clinical Management
- Extensive knowledge of the Aged Care Quality Standards and ability to address Audit requirements
- Registration with AHPRA as a Registered Nurse
- A passion and empathy for working with older residents and their family
- Attention to detail and good communication skills
- Full working rights in Australia
- National Police Clearance
- Influenza vaccination and covid vaccinations
- NDIS Screening
