Clinical Care Coordinator – Aged Care

  • Permanent full time Monday to Friday with flexible day hours
  • Residential Aged Care
  • Perth Metro area – onsite parking
  • Competitive Hourly Rates from $60 – $64ph + Super

 

The Role:

  • Coordinate and oversee the delivery of high-quality clinical care in line with aged care standards and organisational policies
  • Conduct comprehensive clinical assessments and develop, implement, and regularly review individualised care plans
  • Monitor residents’/clients’ health status, ensuring timely intervention and escalation of clinical concerns
  • Lead and support nursing and care staff to ensure safe, evidence-based practice and continuity of care
  • Manage medication administration systems, audits, and compliance with legislative requirements
  • Collaborate with GPs, allied health professionals, families, and external providers to optimise care outcomes
  • Ensure compliance with Aged Care Quality Standards, accreditation requirements, and clinical governance frameworks
  • Oversee and participate in clinical audits, risk management, and quality improvement initiatives
  • Provide education, mentoring, and clinical guidance to staff to build capability and maintain best practice
  • Maintain accurate and up-to-date clinical documentation and reporting, including incident management and care evaluations. 

  

The successful candidate will have:

  • Experience with Clinical Management
  • Extensive knowledge of the Aged Care Quality Standards and ability to address Audit requirements
  • Registration with AHPRA as a Registered Nurse
  • A passion and empathy for working with older residents and their family
  • Attention to detail and good communication skills
  • Full working rights in Australia
  • National Police Clearance
  • Influenza vaccination and covid vaccinations
  • NDIS Screening
