The Role
An exciting opportunity is available for an experienced Intake and Assessment Coordinator to join a multidisciplinary mental health team in Central Queensland.
As the first point of contact for clients referred through the Head to Health Phone Line and other referral pathways, you will play a key role in ensuring clients receive a welcoming, supportive, and seamless entry into services. You will coordinate care, conduct intake and assessment processes, provide brief interventions, and work collaboratively with internal teams and external partners to support positive client outcomes.
This role is ideal for a mental health professional who enjoys working collaboratively, building community connections, and supporting individuals through their recovery journey.
Key Responsibilities Include:
  • Acting as the primary point of contact for clients accessing the service
  • Conducting intake, assessment, and care coordination activities
  • Providing direct client engagement, brief interventions, and aftercare support
  • Coordinating person-centred care plans and facilitating service transitions
  • Collaborating with internal teams and external stakeholders to deliver integrated care
  • Building and maintaining relationships with community partners and lived experience networks
  • Supporting clients to navigate mental health services and access appropriate supports
  • Maintaining accurate documentation and ensuring compliance with organisational and legislative requirements

What We’re Looking For
  • Demonstrated experience working within mental health and/or Alcohol and Other Drug (AOD) settings
  • Strong understanding of mental health systems, legislation, and brief intervention frameworks
  • Experience coordinating care and supporting individuals with complex needs
  • Ability to work autonomously while contributing effectively within a multidisciplinary team
  • Strong stakeholder engagement and partnership-building skills
  • Excellent communication, organisational, and problem-solving abilities
  • Commitment to recovery-oriented and person-centred practice
Essential Requirements
  • Qualifications in Psychology, Social Work, Mental Health Nursing, Counselling, or a related discipline
  • Current registration and/or membership with a relevant professional body (preferred)
  • Current driver’s licence
  • Ability to obtain and maintain relevant screening clearances
