About the Company
Our client is a well-respected Aboriginal Medical Service dedicated to delivering culturally appropriate, community-led healthcare to Aboriginal and Torres Strait Islander peoples. With a strong focus on preventative health, chronic disease management, and improving community health outcomes, they provide comprehensive primary healthcare services in a supportive and inclusive environment.
Due to ongoing service demand, they are seeking an experienced Primary Health Care Nurse to join their team in either a locum or permanent capacity.
The Role
This is a rewarding opportunity for a compassionate and experienced Primary Health Care Nurse who is passionate about delivering culturally safe and patient-centred healthcare.
Working Monday to Friday, you will play a key role in supporting the health and wellbeing of Aboriginal and Torres Strait Islander patients through the delivery of quality primary healthcare services.
Key responsibilities include:
- Conducting patient assessments, health checks, and care planning
- Delivering immunisations, wound care, and chronic disease management programs
- Supporting preventative health initiatives and health promotion activities
- Providing patient education and culturally appropriate healthcare advice
- Maintaining accurate clinical documentation and electronic medical records
- Collaborating with General Practitioners, Aboriginal Health Practitioners, Allied Health Professionals, and the wider healthcare team
You will contribute to a holistic model of care that supports positive health outcomes and strengthens community wellbeing.
What We’re Looking For
To succeed in this role, you will ideally have:
- Current AHPRA registration as a Registered Nurse
- Previous experience within Primary Health Care, General Practice, Community Health, or Aboriginal Health Services (Essential)
- Current Immunisation Certificate (Essential)
- Demonstrated understanding of, and commitment to, culturally safe healthcare practices
- Strong clinical assessment and patient management skills
- Experience with electronic medical record systems and clinical software
- Excellent communication and interpersonal skills
- The ability to build positive relationships with patients, families, and community members
- A genuine commitment to improving health outcomes for Aboriginal and Torres Strait Islander communities
