Support at Home – Home Care Provider in Melbourne
We are seeking an experienced Home Care Manager / Registered Nurse with strong senior clinical and leadership experience to support a highly regarded home care business across two Melbourne regions. This is an exciting opportunity for an accomplished clinical leader to step into a key advisory and leadership role, supporting the franchise team through the next stage of business development and improvement. We are looking for someone who combines strong clinical governance with practical operational leadership, and who has proven experience leading change, improving processes and building high-performing teams.
The Opportunity:
As the Home Care Manager / Registered Nurse, you will act as a valued clinical advisor and operational leader, working closely with the franchise leadership team to strengthen service delivery, clinical governance and client outcomes. You will play a key role in reviewing current practices, identifying opportunities for improvement and implementing practical, sustainable processes that support safe, consistent and high-quality care.
Key Responsibilities:
- Support the team with day-to-day operating processes, performance and accountability.
- Review clinical practices, workflows and processes, identifying gaps and opportunities for improvement.
- Assess clinical governance and client care risks, ensuring appropriate actions and escalation processes are in place.
- Establish clear, practical processes that support consistent service delivery and quality client care.
- Review client services to ensure they remain appropriate, responsive and aligned with assessed needs.
- Strengthen the integration of clinical services within overall client care, ensuring clinical care is embedded within broader service delivery.
- Establish a structured approach to client budget and service reviews, ensuring clients receive the right mix of services to support their needs, safety, independence and quality of life.
- Work closely with the franchise leadership team to implement a clear action plan, with defined priorities, responsibilities and measurable outcomes.
- Provide clinical leadership, mentoring and guidance to the wider team.
- Identify and implement sustainable improvements across clinical, operational and workforce practices.
About You:
You will be an experienced and confident clinical leader who can balance operational and workforce demands with a strong focus on client outcomes, quality and clinical governance.
Ideally, you will bring:
- Senior Registered Nurse experience combined with Practice Manager, Clinical Manager or similar leadership experience.
- Experience within community aged care, home care or a multidisciplinary healthcare environment.
- Strong leadership capability, with the confidence to support teams and hold people accountable for delivery and performance.
- A client-centred approach combined with a strong understanding of clinical governance and risk.
- Proven experience building stable, effective teams that consistently deliver safe, responsive and high-quality care.
- Experience analysing an existing service, identifying gaps and implementing sustainable processes and improvements.
- Strong change management and continuous improvement skills.
- Excellent communication, stakeholder management and problem-solving capabilities.
- Current AHPRA registration.
- Current National Police Clearance.
