4 Week Contract – Community Registered Nurse

  • Be a part of the Support at Home Team
  • 4 Weeks – Full time
  • Monday to Friday 8.30am – 4.30pm
  • Perth and surrounding area – company car provided for use to clients homes
  • Salary dependent on experience with Casual Loading.

The Role:

  • Support clients in their homes and provide clinical support to over 65 year old persons.
  • Support at home under the SAH funding package
  • Case load to provide assessment, coordination of client services, develop support plans, review current plans and adapt to their changing needs.
  • Implement and monitor compliance required of the Aged Care Quality Standards
  • Liaise with medical, allied health, pharmacies, service providers and equipment providers to ensure a holistic, comprehensive delivery of care.
  • Document and report care interventions, observations, changes in needs and consumer feedback in an accurate, objective and efficient manner.
  • Provide clinical advice and education to persona l care staff to ensure appropriate care.
  • Scope of work also includes Male and Female Catheterization, wound care, compression bandages, continence assessment and PAS assessments.

The successful candidate will have:

  • Registration with AHPRA as a Registered Nurse
  • Experience in home care or residential aged care, Hospital environment.
  • Drivers License
  • A passion and empathy for working with older residents and their family
  • Attention to detail and good communication skills
  • Full working rights in Australia
  • National Police Clearance
  • Proof of Influenza vaccination and covid vaccinations
