Care Manager

  • Care Manager / Nurse located Perth South
  • 6 mth Fixed Term Contract – Dynamic role
  • $45 – $50phr + Super + Fully Maintained Car + Fuel Card

As a Care Manager / Community Nurse you will have the best combination of working in the office and out in the community. You will enjoy being part of a collaborative and inclusive team working with the broader home and community team. They have an outstanding reputation, are highly valued in the community sector and are committed to delivering person centred care and best practice to their 60 clients located in Perth South and surrounding areas.

Day to day duties will include:

  • Client management and conducting client care assessments
  • Supervision and management of support workers
  • Ensuring full compliance with organisation and care standards
  • Community engagement and overseeing rostering and budgets
  • Onboarding and training of support workers
  • Community nursing visits

To be successful:

  • Current AHPRA registration
  • Experience working in Home/Community Care
  • In-depth knowledge of Home Care Package funding
  • Strong clinical assessment skills
  • Excellent engagement and communication skills
  • Amazing customer service, friendly approach, compassion and values focused
  • Current WA driver’s licence
  • National Police Clearance within the last 6 months
