Care Partner Aged Care – Part time 32 hours

- Support at Home Care Partner – Open to Cert III through to RN experience
- Experience working in Home Care as a Care Partner, case manager or client coordinator
- Permanent PART TIME – 32 hours per week (working over 5 days)
- Hours are flexible through the day approx 10am – 4.30pm
- Competitive rates dependent on experience plus super
- Salary packaging Available – $18,550
The Role:
- Oversee a caseload within the community, completing thorough clinical assessments.
- Create and deliver tailored care plans that reflect client goals and funding requirements.
- Deliver clinical services such as wound management, medication administration, and support for chronic conditions.
- Supervise and guide a team of Support Workers to ensure safe, high-quality care outcomes.
- Work in partnership with clients, their families, allied health providers, and community organisations.
- Track budgets and maintain adherence to Aged Care Quality Standards and internal policies.
The successful candidate will have:
- Open to seeing candidates with Cert III through to RN AHPRA experience
- Experience as a and Care Partner, Case manager or Client coordinator
- Strong knowledge of Home Care Packages and Aged Care Quality Standards.
- Excellent communication and organisational skills.
- Current driver’s licence with access to a car.
- National Police Clearance and Flu vaccination (no older than 6 months).
