Clinical Manager – Support at Home
We are seeking an experienced Care Manager – Support at Home to oversee the delivery of high-quality, person-centred care to clients within a growing aged care organisation. This is a key role for an experienced aged care professional who can lead care coordination, support client outcomes, and ensure services are delivered safely, effectively and in line with the Support at Home program requirements.

- Permanent full-time opportunity Monday to Friday
- Aged care organisation – Perth Metro Based with parking onsite
- Competitive remuneration package $130k – $140k plus super
- AHPRA qualified Registration required with minimum 2-3 years of experience
The Role:
- Lead the effective delivery of Support at Home services, ensuring high-quality, person-centred care that supports clients to remain independent at home.
- Oversee the end-to-end coordination of client services, including assessments, care planning, service implementation and ongoing reviews.
- Build strong relationships with clients, families, support workers, health professionals and external stakeholders.
- Monitor client needs and service outcomes, identifying opportunities to adjust care plans and improve the client experience.
- Ensure services are delivered in line with Aged Care legislation, quality standards, funding requirements and organisational policies.
- Provide leadership, guidance and support to care and support teams to ensure consistent, high-quality service delivery.
- Work collaboratively with internal teams to manage referrals, client transitions, risks, incidents and complex care requirements.
- Maintain accurate client documentation, reporting and records while ensuring compliance and effective case management.
- Support the financial and operational management of client packages, ensuring services are delivered within allocated funding.
- Contribute to continuous improvement initiatives that enhance service quality, client outcomes and organisational performance.
- Identify and manage risks, escalating complex clinical, safeguarding or service issues where required.
- Promote a positive culture focused on choice, dignity, independence and quality of life for clients receiving Support at Home services.
The Successful Applicant:
- Current AHPRA registration as a Registered Nurse (RN).
- Current National Police Clearance and ability to meet all relevant aged care screening requirements.
- Demonstrated experience in aged care, community care or care management.
- Strong understanding of Support at Home, care planning and client-centred service delivery.
- Proven ability to coordinate services and manage a diverse client caseload.
- Strong knowledge of Aged Care Quality Standards, legislation and compliance requirements.
- Excellent communication and relationship-building skills, with the ability to work effectively with clients, families and stakeholders.
- Strong organisational and time-management skills, with the ability to manage competing priorities.
- Demonstrated ability to identify and manage risks, complex care needs and safeguarding concerns.
- Strong leadership and problem-solving skills, with a collaborative and solutions-focused approach.
- Commitment to delivering high-quality, person-centred care that promotes choice, dignity, independence and positive client outcomes.
