Community Registered Nurse

- Minimum 4 Week Contract with ability to go permanent full time
- Full Time – Monday to Friday
- Car Available to pick up and use daily
- Must have aged care experience – Community in home experience (not residential)
- (Primarily Perth Metro – North and South of the River) there is some clients in Mandurah but it would be 1 day every few weeks to travel there
- Base Hub – Innaloo
- Hourly rate dependent on experience
The Role:
- Conduct comprehensive clinical assessments and develop person-centred care plans.
- Deliver high-quality nursing care to clients in their own homes in accordance with best practice standards.
- Administer medications, wound care, catheter care and other clinical treatments as required.
- Monitor clients’ health and wellbeing, identifying changes in condition and escalating concerns appropriately.
- Coordinate care with GPs, allied health professionals, support workers and other members of the multidisciplinary team.
- Complete accurate and timely clinical documentation, assessments and progress notes.
- Educate and support clients, families and carers to promote independence and achieve positive health outcomes.
- Ensure services are delivered in accordance with AHPRA standards, aged care legislation and organisational policies.
- Participate in case conferences, clinical reviews and ongoing care planning.
- Contribute to quality improvement initiatives and maintain compliance with clinical governance requirements.
- Build strong relationships with clients and their families through compassionate, person-centred care.
- Travel within the Perth metropolitan area to provide community-based nursing services.
The successful candidate will have:
- Current AHPRA registration as a Registered Nurse.
- Previous experience in community, aged care or home care nursing essential.
- A current National Police Clearance (within 6 months)
- A current NDIS Worker Screening Clearance
- Current First Aid and CPR certification
- A valid Australian Driver’s Licence a
- Full Australian working rights.
- Experience using electronic clinical documentation systems
- Knowledge of the Aged Care Quality Standards and contemporary community care practices.
