Occupational Therapist

We are seeking a motivated Occupational Therapist to join two of our clients multidisciplinary team. In this role, you will support clients to improve and maintain their independence in daily living activities through comprehensive assessment, individualised treatment planning and evidence-based interventions. You will work collaboratively with clients, families and other health professionals to recommend appropriate strategies, equipment and supports that enhance function and quality of life. The Role:

* Conduct comprehensive functional assessments to evaluate clients’ physical, cognitive, emotional and social abilities and identify barriers to independence

* Develop, implement and review individualised, goal-oriented treatment and intervention plans using evidence-based occupational therapy practice

* Support clients to improve independence in activities of daily living including personal care, mobility, home management and community participation

* Assess, recommend and prescribe assistive equipment, adaptive technologies and environmental modifications to enhance safety and function

* Collaborate closely with clients, families, carers and multidisciplinary health professionals to ensure coordinated and holistic care planning

* Monitor, evaluate and adjust intervention plans based on client progress, changing needs and treatment outcomes

* Provide education, training and practical strategies to clients, families and support staff to optimise functional outcomes and independence

* Maintain accurate, timely and professional clinical documentation in line with organisational and regulatory requirements

* Contribute to risk assessments, safety planning and prevention strategies including falls prevention and functional decline management

* Support discharge planning and transition of care to ensure continuity of services and ongoing client support

* Promote client wellbeing, participation and quality of life through a strengths-based and person-centred approach

* Participate in multidisciplinary meetings, case conferences and continuous improvement initiatives to enhance service delivery

The Successful candidate will have:

  • Bachelor’s degree in Occupational Therapy (or equivalent qualification) and current AHPRA registration
  • National Police Clearance (within 6 months)
  • Strong clinical knowledge in assessment, treatment planning and evidence-based intervention
  • Experience supporting clients with diverse physical, cognitive and psychosocial needs
  • Ability to develop and implement individualised, goal-focused therapy programs
  • Competence in prescribing assistive equipment and recommending home/environmental modifications
  • Excellent communication and interpersonal skills with a client-centred approach
  • Proven ability to work effectively within multidisciplinary teams
  • Strong organisational skills with the ability to manage a varied caseload
  • Sound clinical documentation and report writing skills
  • Commitment to continuous professional development and best practice standards
  • Understanding of risk management, safety and quality improvement principles
  • Empathy, professionalism and a genuine passion for improving client independence and wellbeing
