• Residential Aged Care and Community Aged Care Positions Available
• We have roles in both Perth Metro and Regional Locations
• Permanent Full time and Part time
• Salary starting at $85k – $116k plus Super
• Not for Profit Organisations with salary packaging available

The Role:

• Conduct comprehensive functional, environmental, and equipment assessments for residents and clients.
• Develop and implement individualised therapy and rehabilitation plans.
• Recommend and prescribe assistive equipment, mobility aids, and home modifications.
• Support residents in maintaining independence, safety, and quality of life.
• Work collaboratively with nurses, physiotherapists, care staff, and families to deliver holistic care.
• Maintain accurate documentation in line with Aged Care Quality Standards and organisational policies.
• Contribute to manual handling, falls prevention, and wellbeing programs

• Complete therapy assessments and respond to clinical referrals
• Plan and implement individual or group therapeutic activities, whilst working with an interdisciplinary team to maintain an effective referral system and coordinate therapy implementation
• Maintain appropriate clinical documentation and information systems
• Contribute to a multidisciplinary program of continuous improvement
• Synthesise information, through observation and written documentation, to provide recommendations that are relationship centred and informed by evidence based best practice
• Undertake a range of assessments that help to understand the holistic experience of the person

The successful candidate will have:

• Current full, unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Occupational Therapist
• Relevant experience in an aged care environment
• Be relatable and able to share knowledge in a way that supports change at all levels
• High level of negotiation, communication and relationship building skills
• Excellent problem solving and decision-making skills
• Able to manage a complex workload and adapt to fast changing priorities and environments
• Is customer focussed and works effectively with a broad range of stakeholders
• Demonstrates effective communication, strong verbal and written skills, and interpersonal skills
• Covid and Flu Vaccination
