Join a supportive Home Care team committed to helping older Australians live safely, independently and confidently in their own homes. In this role, you’ll work alongside clients, their families, support workers and other health professionals to ensure each person receives the right level of support as their circumstances and care needs evolve.

About the role

We’re looking for a Registered Nurse who brings confidence, empathy and strong clinical judgement to their work. You’ll be someone who enjoys getting to know clients, takes pride in coordinating quality care and can confidently step in when decisions need to be made. You’ll manage a caseload of clients with more complex care requirements, taking responsibility for assessments, care planning, reviews and ongoing coordination. While your nursing background is essential, this position is primarily focused on care management rather than routine clinical visits.

Your days will include:

  • Meeting new clients in their homes, completing assessments and coordinating their transition into services.
  • Developing individualised care plans that reflect each client’s goals, preferences and changing needs.
  • Reviewing services and care plans regularly, responding promptly when a client’s circumstances change.
  • Monitoring package funding and ensuring available resources are being used effectively and in line with client priorities.
  • Maintaining regular contact with clients and their families to ensure they remain informed and supported.
  • Coordinating care with allied health professionals, healthcare providers and other community services.
  • Providing guidance and direction to support workers when care requirements or client circumstances change.
  • Undertaking clinical nursing duties when required, including wound management and health assessments.
  • Ensuring client information, assessments and care documentation remain accurate and current.

What you’ll receive

  • $53 per hour + Superannuation
  • Company vehicle, fuel card and laptop supplied.
  • Monday to Friday, generally 9.00am-5.00pm, with some flexibility around start and finish times.
  • The opportunity to develop meaningful, ongoing relationships with clients and see the impact of your care coordination over time.
  • A varied role combining client engagement, care planning, coordination and hands-on nursing when required.

About you

You may currently be working as a Care Manager, Care Partner, Community RN or in a similar Home Care position. You’ll understand that great care management is about more than completing paperwork – it’s about listening carefully, identifying what matters to each client and making sure the right support is available when it’s needed.

To be successful, you’ll have:

  • Current AHPRA registration as a Registered Nurse.
  • Previous experience in care management, client reviews or Home Care Packages.
  • The ability to confidently coordinate support workers and collaborate with allied health professionals and external providers.
  • Strong clinical assessment skills and the confidence to provide hands-on nursing care when required.
  • Excellent communication skills and the ability to have clear, compassionate conversations with clients and families.
  • Strong organisational skills and accurate documentation practices.
  • Confidence using electronic client management and record-keeping systems.
  • A current driver’s licence and the ability to travel to clients’ homes.
  • Current First Aid and CPR certification, or the willingness to obtain these.
