Senior Clinical Care Partner – Support at Home

Support at Home has changed how community aged care is delivered, and our client is looking for a Senior Clinical Care Partner who wants genuine leadership scope shaping how care is planned, funded and delivered so older Australians can stay independent and confident in their own homes.
About the role:
Reporting to the Community Manager and leading a team you’ll provide clinical leadership and oversight across the Support at Home Program and its pathways, ensuring clients receive safe, coordinated, goal-focused care. This role is based approximately 30kms out of central Brisbane.
- Lead the program from end to end guiding complex cases, facilitating case review and reflective practice, and embedding evidence-informed care.
- Oversee assessment, individualised care and goal planning, and the integration of nursing, allied health, assistive technology and home modifications
- Manage referrals and intake, triage and assign cases, and drive timely care plan reviews in partnership with clients as their needs evolve.
- Hold clinical governance and quality standards and participate in the Clinical Excellence Team, reviewing incidents within scope, auditing, and driving continuous improvement in line with the Strengthened Aged Care Quality Standards.
- Oversee client budgets and funding within delegated authority, approve invoices, and support your team to maintain financial governance, working closely with Procurement and the Home Care Finance Business Partner.
- Mentor and develop the team and promote Support at Home services in the community.
To be successful:
- Registered Nurse with current AHPRA registration.
- Sound knowledge of the Support at Home Program and Strengthened Aged Care Quality Standards including compliance, funding and reporting requirements.
- Strong clinical capability across assessment, referrals, case management and goal-based care planning and community aged care experience highly regarded.
- Proven team leadership, with a track record of building inclusive, high-performing teams.
- Excellent communicator and credible with clients, families, staff and external stakeholders.
- Confident coordinating competing priorities, using client management systems, and maintaining accurate, timely documentation.
- Sound judgement in complex situations, able to identify risk, implement solutions, and lead a team through change.
- Current National Police Clearance
- Current Drivers Licence with comprehensive insurance
- First Aid and CPR certificate
- Covid and Flu vaccination desirable
