• Support at Home Care Partner – RN qualified
• Experience working in Home Care and has an AHPRA
• Permanent PART TIME – 32 hours per week (working over 5 days)
• Hours are flexible through the day approx 10am – 4.30pm
• Competitive rates dependent on experience plus super
• Salary packaging Available – $18,550

The Role:

• Manage a community-based caseload and conduct comprehensive clinical assessments.
• Develop and implement individualised care plans aligned with consumer goals and funding guidelines.
• Provide clinical care including wound care, medication management, and chronic disease support.
• Lead and mentor a local team of Support Workers, ensuring safe and effective service delivery.
• Collaborate with clients, families, allied health professionals, and community organisations.
• Monitor budgets and ensure compliance with Aged Care Quality Standards and organisational policies

The successful candidate will have:

• Registered Nurse with current AHPRA registration

• Experience as a RN and Care Partner, Case manager or Client coordinator
• Strong knowledge of Home Care Packages, consumer directed care, and Aged Care Quality Standards.
• Excellent communication, leadership, and organisational skills.
• Ability to work independently and manage a small team.
• Current C Class driver’s licence with access to a car.
• To work in aged care, we require employees to receive annual flu vaccinations and provide proof of current National Police Clearance (no older than 6 months).
